A friend of mine used to belong to Kaiser's HMO. In one of many mishaps, he discovered that two doctors had each prescribed the same drug, presumably under different names, so my friend was taking a double dose. What I don't understand is this: pharmacy software has been around for AGES to guard against such things. Why didn't the software catch it? Why didn't any of the doctors or nurses catch it? HUGE mistake--and very preventable with just a little attention to the patient's history.
