It's a salad with chicken and some kind of peanut (I think) sauce/dressing.  I have no idea why they call it a "Love Salad," but that's what it's called.

Yes.  They usually have plain chicken, but teriyaki chicken is an option for an additional cost.

I don't think we have anymore Happy Teriyaki's in Tacoma.  We used to have several.
